Application Instructions

1. Complete and sign the application form.

2. Obtain the endorsement of your principal.

3. Include all required attachments.

4. Mail application to:

Reimbursement of Travel Expenses

In performing expenditure responsibility, the Licking County Foundation will reimburse recipients up to the amount of the grant for travel, accommodations, meals, and other related approved expenses. A copy of the receipts to be reimbursed, as well as the written report, must be submitted to the Foundation prior to reimbursement. The Foundation must issue payment to the school or school district, which will then reimburse the recipient. The recipient will receive notification when the reimbursement is sent to the school or school district.

The Tibbie Leslie Travel Grant application deadline is Tuesday, January 10, 2012 by 5:00 pm.
